Leodis McKelvin returned a punt 79 yards to give the Bills an early 7-0 as they held off the Dolphins, 19-14.
Rian Lindell hit four field goals for Buffalo, who are now 4-and-6 on the season.
Ryan Fitzpatrick threw for 168 yards for the Bills.
Ryan Tannehill tossed for 141 yards, a score and a pair of interceptions for Miami, which dropped to 4-and-6.
